Introduction: Pneumonia in patients admitted to Intensive Care Units (ICUs) is a significant cause of hospital-related morbidity and mortality, particularly in individuals undergoing mechanical ventilation, those who are immunocompromised, or those with chronic diseases. The clinical complexity of these patients, combined with the potential for infection by multidrug-resistant microorganisms, complicates diagnosis, therapeutic decision-making, and prognostic assessment. Objective: To analyze the primary challenges related to the diagnosis, treatment, and prognosis of pneumonia in critically ill patients admitted to the ICU, considering the pathophysiological, microbiological, and clinical aspects involved in managing this condition. Methodology: This is a descriptive narrative literature review based on clinical guidelines, scientific consensus statements, and relevant publications concerning community-acquired pneumonia requiring intensive care, hospital-acquired pneumonia, and ventilator-associated pneumonia. Discussion: Diagnosing pneumonia in critically ill patients requires the integration of clinical data, laboratory parameters, diagnostic imaging, and microbiological investigation. The overlap of clinical manifestations with other conditions—such as pulmonary edema, acute respiratory distress syndrome (ARDS), and atelectasis—can compromise diagnostic accuracy. Treatment involves empirical antibiotic therapy, guided by the severity of the illness, risk factors for antimicrobial resistance, and the institutional epidemiological profile, with subsequent adjustments based on microbiological results. Prognosis depends on factors such as the severity of respiratory failure, the presence of septic shock, comorbidities, the appropriateness of initial treatment, and the occurrence of organ dysfunction. Conclusion: Managing pneumonia in the ICU requires a multidisciplinary approach, timely diagnosis, rational antibiotic therapy, individualized respiratory support, and continuous monitoring of the clinical response. The implementation of prevention strategies, the judicious use of antimicrobials, and the early identification of complications are essential for reducing morbidity and mortality and improving clinical outcomes.
