Abstract

Tuberculosis remains an important public health problem in Brazil, especially in territories marked by social inequalities and structural vulnerabilities. This chapter aimed to critically discuss Directly Observed Treatment within Brazilian Primary Health Care, emphasizing its relationship with territorialization of care, therapeutic adherence and strengthening of healthcare networks in the Brazilian Unified Health System. This is a theoretical-reflective study based on national and international scientific literature related to tuberculosis control, therapeutic adherence and Primary Health Care organization. The findings indicate that Directly Observed Treatment goes beyond a strictly biomedical dimension and constitutes an important territorial device for healthcare production, requiring bonding, welcoming practices, longitudinal care and intersectoral articulation. Furthermore, poverty, social stigma, food insecurity, psychological distress and fragile social support networks directly interfere with therapeutic continuity and the effectiveness of actions developed by Family Health Strategy teams. It is concluded that strengthening tuberculosis control actions requires expanding the problem-solving capacity of Primary Health Care, valuing multiprofessional work and consolidating public policies committed to comprehensive care and reduction of social inequalities.
