IMUNOFENOTIPAGEM DE LINFÓCITOS T E A PATOGÊNESE DA PERDA ÓSSEA EM PACIENTES COM HIV SOB SUPRESSÃO VIRAL

Resumo

Com a introdução da terapia antirretroviral combinada (TARV), a expectativa de vida das pessoas vivendo com HIV/AIDS (PVHA) aumentou significativamente, transformando a infecção em uma condição crônica. Contudo, o envelhecimento dessa população trouxe à tona uma incidência elevada de comorbidades, como a perda óssea precoce, exacerbada pela inflamação sistêmica persistente e ativação imune crônica. A avaliação dos fenótipos de subpopulações de linfócitos T CD4+ e CD8+ estão associados à redução da densidade mineral óssea (DMO) em pacientes vivendo com HIV com carga viral controlada.  A avaliação dos exames de densitometria óssea (DXA) e análises de imunofenotipagem por citometria de fluxo, é viável para a detecção dos linfócitos diretamente realacionado a redução da densidade óssea nos pacientes HIV. É essencial investigar os marcadores de regulação (PD-1, CTLA-4), ativação (HLA-DR), proliferação (Ki-67) e produção de citocinas. Os pacientes HIV em controle viral, é observado uma alta prevalência de alterações ósseas, como osteopenia e osteoporose. Os estudos demonstram que pacientes com osteopenia apresentaram maior atividade proliferativa em linfócitos T e uma quantidade reduzida de células regulatórias (CD4+, CD3+, CTLA-4+ e CD8+, CD3+ e PD-1+) comparados aos pacientes com DMO normal. Além disso, é observado um aumento na expressão de marcadores de ativação (HLA-DR e CD69) e desequilíbrio na produção de citocinas inflamatórias, como TNFα e IFNy. Entende-se que perfis específicos de linfócitos circulantes e a disfunção regulatória desempenham um papel relevante na patogênese da perda óssea em PVHA (Pacientes Vivendo com HIV Controlado).
