MANEJO DA HIPERTENSÃO INTRACRANIANA NO TRAUMATISMO CRÂNIO‑ENCEFÁLICO GRAVE: ATUALIZAÇÃO BASEADA EM EVIDÊNCIAS

A hipertensão intracraniana (HIC) representa uma das principais causas de lesão secundária evitável e de óbito no traumatismo crânio‑encefálico grave (TCEg), definida como pressão intracraniana (PIC) sustentada acima de 22 mmHg por período superior a cinco minutos. Fundamentada na hipótese de Monro‑Kellie, a elevação da PIC reduz a pressão de perfusão cerebral (PPC = PAM – PIC), predispõe à isquemia, herniação e morte neuronal. Este trabalho revisa as evidências vigentes até 2025, com foco em critérios de monitorização, metas alvo, medidas gerais, terapias escalonadas, intervenções cirúrgicas e perspectivas de neuromonitoramento multimodal. Conclui‑se que a abordagem estruturada e individualizada, guiada por PIC, PPC e integridade da autorregulação, reduz mortalidade e melhora o prognóstico funcional.
