KIDNEY-BONE MARROW CROSSTALK IN AGING: PATHOPHYSIOLOGICAL INTERACTIONS AND THE SPECTRUM OF ANEMIA IN GERIATRIC CHRONIC KIDNEY DISEASE

Global population aging is an established demographic phenomenon, bringing with it a significant rise in the prevalence of chronic non-communicable diseases. Among these, Chronic Kidney Disease (CKD) plays a prominent role in the elderly population, stemming from both physiological renal senescence and the lifelong accumulation of comorbidities such as arterial hypertension and diabetes mellitus. The progression of renal dysfunction in this age group exerts a profound systemic impact, with direct and significant repercussions on hematological homeostasis. The most frequent and clinically relevant alteration is anemia—a condition of multifactorial etiology—where the central mechanism is a relative deficiency in erythropoietin production by the damaged kidney; this is exacerbated by chronic inflammatory states ("inflammaging") leading to iron sequestration, as well as reduced erythrocyte survival within the uremic environment. Beyond the red blood cell lineage, uremia can induce qualitative platelet dysfunctions—increasing bleeding risk—and alter leukocyte responses, thereby contributing to immunosenescence. The presence of these hematological abnormalities in elderly patients with CKD is associated with poorer outcomes, including heightened cardiovascular risk, cognitive decline, and physical frailty. Therefore, early recognition and integrated management of the kidney-hematopoiesis interface are essential for maintaining quality of life and reducing morbidity and mortality in this vulnerable population.
