ACUTE VERSUS CHRONIC REJECTION IN PEDIATRIC PATIENTS: BIOMARKERS AND NEW DIAGNOSTIC APPROACHES

This chapter aims to analyze the mechanisms involved in acute and chronic rejection in pediatric transplant patients, emphasizing the role of biomarkers and new diagnostic approaches. It is a descriptive literature review based on studies by authors such as Smith (2021), Johnson and Lee (2020), and Oliveira et al. (2023), which investigate immunological differences between rejection types and technological advances in early detection. The results indicate that acute rejection is characterized by rapid clinical manifestations linked to cellular activation, while chronic rejection is associated with persistent inflammatory processes and tissue fibrosis. The literature shows that biomarkers such as IL6, troponin, and microRNAs have the potential to improve outcome prediction and guide therapeutic decisions. It is concluded that the use of advanced diagnostic tools, combined with individualized monitoring, can enhance graft survival and improve the quality of life of children. The integration of diagnostic protocols based on biomarkers in pediatric transplant services is recommended.
