Endocrine therapy is one of the main therapeutic approaches for women with hormone receptor-positive breast cancer, significantly reducing the risk of recurrence and mortality. However, adherence to and persistence with treatment remain major challenges, as adverse effects, the prolonged duration of therapy, and psychosocial factors may compromise treatment continuity. In this context, the present study aimed to analyze the scientific evidence regarding interventions designed to improve adherence to endocrine therapy among women with hormone receptor-positive breast cancer. An integrative literature review with a qualitative approach was conducted using the PubMed/MEDLINE, Virtual Health Library (VHL), Scopus, and Web of Science databases. Original studies published in Portuguese, English, and Spanish that investigated interventions aimed at improving adherence to or persistence with endocrine therapy were included. The analyzed studies demonstrated that educational strategies combined with continuous follow-up by healthcare professionals yielded the most consistent results in promoting treatment adherence and persistence. Psychosocial interventions, particularly those delivered through telehealth, improved self-efficacy, coping ability, quality of life, and symptom management. In contrast, interventions based on digital technologies produced heterogeneous results, with varying benefits regarding medication adherence. Overall, the evidence suggests that multifaceted interventions integrating health education, continuous professional support, and technological resources are more likely to enhance adherence to endocrine therapy and improve clinical outcomes among women with hormone receptor-positive breast cancer.
