Spinal cord injury (SCI) associated with vertebral fractures represents a condition with high morbidity and mortality, with significant functional, social, and economic impact. This article reviews the main orthopedic aspects related to the pathophysiology, classification, diagnosis, and management of these injuries. The mechanisms of trauma, the most commonly used classification systems, conservative and surgical therapeutic strategies, as well as current complications and perspectives are discussed. Despite advances in surgical techniques and intensive care support, significant limitations in neurological recovery persist, highlighting the need for a multidisciplinary approach and early intervention.
