OPIOID-FREE ANESTHESIA: FROM CHALLENGING ANESTHETIC PRACTICE TO BREAKING PARADIGMS

The Opioid-Free Anesthesia (OFA) has proven to be an alternative to opioid-based anesthesia (OA), capable of reducing postoperative side effects (nausea, vomiting, and hyperalgesia) related to opioids, as well as reducing hospitalization time and minimizing clinical and social impacts from the abuse of these substances. The goal is to verify the impacts of OFA compared to conventional anesthesia on postoperative recovery regarding the reduction of side effects and hospitalization time. The methodology used in this Integrative Literature Review study consisted of consulting databases the PubMed/MEDLINE (National Library of Medicine); Scientific Eletronic Library Online (SCIELO); Virtual Health Library (BVS) and Scopus/Web of Science databases were consulted, according to inclusion criteria: review articles (Systematic, Integrative, Narrative and Bibliographic), published between 2022 and 2025, in Portuguese or English. The results obtained include eight selected articles: three integrative reviews; one systematic review (including 142 articles and five randomized retrospective clinical trials – 488 patients aged 18 to 70 years); two literature review articles, one narrative, and one bibliography; all addressing the impacts of opioid-free anesthesia (OFA) in the postoperative period. The conclusion of this study indicates that the side effects, including nausea, vomiting, and pain, can be reduced with the use of opioid-free anesthesia, as well as the length of hospital stay. However, despite promising evidence, this practice lacks more robust research, as the literature is still in the process of consolidation, to strengthen the adoption of OFA by a greater number of anesthesiologists and hospital institutions.
