The COVID-19 pandemic imposed significant challenges on Primary Health Care (PHC), particularly in small municipalities with high socioeconomic vulnerability. Although progress was made in immunizing adults and older adults, childhood vaccination coverage remained below recommended targets, highlighting the need for specific strategies to expand vaccination among children. This study aims to develop an intervention plan to measure and increase COVID-19 vaccination coverage among children in the territory served by the Maria da Conceição Carneiro Family Health Strategy, in the municipality of Bonito de Minas, Minas Gerais, Brazil. This is a descriptive and exploratory study, presented as an experience report, developed within the context of Primary Health Care and structured as a health intervention project. The plan was developed based on the Situational Strategic Planning method, enabling the identification of priority problems and the proposal of feasible actions within the territory. Strategies include community mobilization, strengthening partnerships between the health care team, schools, and local leaders, as well as improving the recording and monitoring of administered doses through the e-SUS and LocalizaSUS systems. An increase in childhood vaccination coverage is expected, along with strengthened public trust in health services and the consolidation of Primary Health Care as a central pillar in disease prevention and the promotion of collective protection.
