This study presents a critical literature review on surgical approaches of mastectomy and breast-conserving surgery in breast cancer treatment, evaluating clinical efficacy, recurrence rates, postoperative complications, and impact on patients’ quality of life. Articles published between 2021 and 2023 were reviewed from databases such as PubMed, Scopus, Google Scholar, and Cochrane, including clinical trials, systematic reviews, and observational studies. Results indicate that both techniques are effective in terms of survival, while breast-conserving surgery offers better aesthetic outcomes, lower psychosocial impact, and higher patient satisfaction. Mastectomy remains indicated for large, multifocal tumors or when radiotherapy is contraindicated. This work emphasizes the importance of personalized surgical treatment, considering tumor characteristics and patient preferences, and provides guidance for evidence-based clinical practice.
