Periodontitis is a chronic inflammatory disease of bacterial origin that affects the tooth-supporting tissues and has the potential to cause systemic repercussions. During pregnancy, hormonal and immunological changes may intensify periodontal inflammatory responses, increasing susceptibility to adverse obstetric outcomes. This chapter aims to analyze, through an integrative literature review, the association between maternal periodontitis and obstetric complications such as preterm birth, preeclampsia, and low birth weight. Evidence indicates that odontopathogenic microorganisms and inflammatory mediators may reach the systemic circulation and the placental environment, contributing to disruptions in maternal–fetal homeostasis. It is concluded that periodontal disease control during prenatal care represents an important strategy for promoting maternal and neonatal health.
