Primary Health Care (PHC) is the preferred entry point to Brazil’s Unified Health System (SUS). This article aims to reflect on the innovations implemented within PHC and the challenges faced in the pursuit of more comprehensive, equitable, and humanized care. Based on literature review and analysis of guidelines from the Ministry of Health, strategies for expanding access, valuing multidisciplinary teams, implementing technologies, and intersectoral policies are discussed. It is concluded that consolidating PHC as the structural axis of the care network depends on continuous investments, team appreciation, and active listening to the population.
