SURGICAL EXTRACTION OF IMPACTED MANDIBULAR THIRD MOLAR: CASE REPORT

This case report aims to describe in detail the surgical procedure of extraction of the impacted lower left third molar (tooth 38), highlighting each operative step, intraoperative management, and postoperative care. The patient presented with recurrent pain in the posterior mandibular region, associated with local inflammation and difficulty in mastication. Radiographic evaluation confirmed horizontal impaction of tooth 38, closely related to the mandibular ramus. The procedure was performed under local anesthesia using an envelope incision, controlled osteotomy, and odontosection, followed by careful tooth removal and copious irrigation with saline solution. The socket was cleaned, and suturing was performed with 4.0 silk thread. Postoperative instructions included antibiotic, analgesic, and anti-inflammatory medication, as well as local care. The patient showed satisfactory healing without significant complications. This report emphasizes the relevance of careful surgical planning, proper use of radiographic tools, and meticulous technical execution to minimize risks and ensure successful extraction of impacted third molars.
