Abstract

Trauma is one of the leading causes of death and disability in the contemporary world, representing a significant public health problem due to its high morbidity and mortality rates and substantial socioeconomic impact. The polytrauma patient requires a rapid, systematic, and multidisciplinary approach, based on internationally validated protocols capable of early identification of potentially fatal injuries and immediate treatment. Initial care is based on primary assessment using the ABCDE trauma protocol, combined with hemodynamic resuscitation, damage control, prevention of the lethal triad, and continuous monitoring. Therapeutic success depends directly on the integration of pre-hospital care, in-hospital stabilization, complementary examinations, and early definition of surgical priorities. This article provides a comprehensive and in-depth review of the fundamental principles of initial care for polytrauma patients, including trauma pathophysiology, clinical assessment, ventilatory support, hemodynamic management, hemorrhagic control, traumatic brain injury, thoracic trauma, abdominal trauma, damage control, fluid resuscitation, massive transfusion, intensive monitoring, and ethical aspects involved in the care of severe trauma.
