Abstract

Antidepressants are a primary pharmacological strategy for treating depressive disorders and various neuropsychiatric conditions. Despite their efficacy, adverse effects can compromise adherence, treatment continuity, and, consequently, therapeutic success. Neuropsychiatric manifestations potentially linked to antidepressant use include drowsiness, insomnia, anxiety, agitation, cognitive changes, emotional blunting, sexual dysfunction, mood alterations, and—in certain individuals—symptoms of activation or the precipitation of manic or hypomanic episodes. Furthermore, the abrupt discontinuation of certain drugs can trigger withdrawal symptoms that may lead patients to abandon treatment. This article discusses the main neuropsychiatric effects that can interfere with antidepressant adherence, the underlying mechanisms, risk factors, and clinical strategies to minimize these events. It is important to note that the presence of symptoms during treatment does not necessarily imply they are caused by the antidepressant, as similar manifestations may stem from the psychiatric disorder itself or other clinical conditions.
