Abstract

Polyendocrine Metabolic Ovarian Syndrome (PMOS) has manifestations that extend beyond the reproductive system, involving metabolic, inflammatory, intestinal, and cellular alterations. This study aimed to analyze the condition’s main pathophysiological interfaces and their repercussions on ovarian function and women’s quality of life. This qualitative, exploratory integrative literature review was conducted in the PubMed/MEDLINE, Scopus, Web of Science, and Embase databases and considered publications from 2021 to 2026. Following the identification, screening, and eligibility stages, 17 studies were included. The studies demonstrated relationships among insulin resistance, hyperandrogenism, gut dysbiosis, inflammation, and altered ovarian function. Mitochondrial alterations, NAD⁺ deficiency, and impaired communication between the oocyte and granulosa cells were also identified and associated with impaired follicular development and oocyte competence. Experimental and clinical studies further indicated potential effects of nutritional interventions, bioactive compounds, and microbiota-targeted strategies. Therefore, an integrated understanding of the mechanisms involved may support more individualized approaches to prevention and care that consider the condition’s metabolic, reproductive, and psychosocial dimensions.
