Introduction: Ankyloglossia, known as “tongue-tie,” is a congenital condition characterized by limited tongue movement due to alterations in the lingual frenulum, which may compromise essential functions such as speech, mastication, swallowing, and breathing. In later stages of life, especially in adolescents and adults, this condition can lead to significant functional and social impacts, making an appropriate therapeutic approach necessary. Objective: To analyze the role of speech-language pathology in the pre- and postoperative periods of lingual frenectomy in late cases, highlighting its importance in the functional rehabilitation of patients with ankyloglossia. Methodology: This is a descriptive and qualitative bibliographic review based on national and international scientific articles obtained from electronic databases such as SciELO and PubMed. Studies published between 2022 and 2025 addressing assessment, intervention, and speech-language rehabilitation in patients undergoing lingual frenectomy were included. Results and Discussion: The analyzed studies show that lingual frenectomy significantly improves tongue mobility, directly reflecting on speech, mastication, and swallowing functions. However, speech-language intervention is essential both in the preoperative period—preparing the musculature and guiding the patient—and in the postoperative period, focusing on myofunctional reeducation, reorganization of functional patterns, and prevention of limitations resulting from compensatory adaptations. Conclusion: It is concluded that speech-language pathology intervention in the pre- and postoperative periods of late frenectomy is indispensable for complete functional rehabilitation, contributing to the improvement of oral functions and quality of life. The integration among healthcare professionals enhances treatment outcomes, highlighting the importance of a multidisciplinary approach.
