A tirzepatida é um agonista duplo dos receptores de GLP-1 (glucagon-like peptide-1) e GIP (glucose-dependent insulinotropic polypeptide), inicialmente indicada para o tratamento do diabetes mellitus tipo 2. Contudo, sua extensão terapêutica para manejo da obesidade e outras condições metabólicas tem ampliado seu uso clínico. Este artigo analisa, de forma crítica e baseada em mecanismos fisiopatológicos, as principais manifestações clínicas associadas ao uso prolongado da tirzepatida, destacando efeitos gastrointestinais, metabólicos, cardiovasculares e potenciais complicações raras, com ênfase na individualização terapêutica e na farmacovigilância.
