Botulinum toxin type A (BT-A), an exogenous high-molecular-weight protein produced by Clostridium botulinum, blocks acetylcholine release at the neuromuscular junction and is the most performed minimally invasive aesthetic procedure in healthy individuals. Systemic lupus erythematosus (SLE) is characterized by loss of immune self-tolerance, B-cell hyperactivation, autoantibody production and immune complex deposition, with an intermittent course between remission and flares. Given this immunological lability, introduction of a bacterial protein with immunogenic potential during active disease may act as a trigger for systemic flare, neutralizing antibody formation and hypersensitivity reactions. This is an integrative review (2020-2025) in SciELO, PubMed and Google Scholar databases. No randomized clinical trials evaluating facial cosmetic safety of BT-A in SLE were found, with literature comprising indirect evidence. Chronic use of corticosteroids and immunosuppressants was found to delay healing of puncture sites and mask secondary infections. It is concluded that facial cosmetic application of BT-A in SLE is only possible in stable disease, clinically and laboratory controlled, through rigorous screening and risk management conducted by the aesthetic pharmacist in an interdisciplinary team with dermatologist and assisting rheumatologist, with documented clearance from the latter.
