THE PHARMACIST'S ROLE IN PROMOTING THE RATIONAL USE OF MEDICINES AND PATIENT SAFETY

This chapter aims to analyze the role of the pharmacist in promoting the rational use of medicines and patient safety, highlighting its importance across different levels of healthcare. This is a narrative literature review based on scientific studies indexed in databases such as SciELO and PubMed, as well as documents from organizations such as the World Health Organization and the Brazilian Ministry of Health. The findings indicate that pharmacists play a crucial role in therapeutic guidance, prevention of drug interactions, monitoring of adverse drug reactions, and health education, directly contributing to the reduction of medication errors and improved treatment adherence. Furthermore, practices such as pharmaceutical care and medication reconciliation have shown a positive impact on healthcare quality and clinical outcomes. It is concluded that the active inclusion of pharmacists in multidisciplinary teams strengthens patient safety and promotes the appropriate use of medicines, being essential for improving healthcare services.
